New Patients
To become a new patient, please have the referring doctors office fax or email us your records (office visit notes, procedure notes, imaging, demographics including insurance information). Our fax number is 410-641-1746. Our email address is berlinpainmanagement.com.
Once we receive all of your records, we will verify your insurance benefits (co-pay, co-insurance, deductible, out of pocket). Our office will reach out to you within 5 business days to schedule your new patient appointment.
What to expect at your first visit:
Please plan to arrive 10 minutes early for your appointment so that our staff may facilitate completion of any required forms, receipt of any required copays and so that our staff may prepare you for your visit with your doctor.
Make sure to bring:

- Completed patient forms
Insurance
We accept most major insurance plans. However, there are a few exceptions. Please call our office to verify if we accept your plan.
The extent of your benefits is determined by your individual policy; depending upon your plan, some procedures may require pre-authorization or may not be covered.

Cancellations
Berlin Interventional Pain Management requires a 24 hours notice when cancelling an office appointment and/or a procedure appointment. Appointments can be cancelled or rescheduled by calling our office. Messages may be left 24 hours a day.
Office Consultations: A cancellation fee of $80.00 will be charged for no-shows and appointments not cancelled in a timely fashion.
Procedures: A cancellation fee of $250.00 will be charged for no-shows and appointments not cancelled in a timely fashion.
